Chronic Care Management Of Cardiovascular Disorders

What is Chronic Care Management?

Chronic Care Management (CCM) programs focus on enhancing communications between the patient and the healthcare provider to provide chronically ill patients with customized healthcare services.

The Benefits of Chronic Care Management

A patient enrolled in a Chronic Care Management program recovers more quickly after a cardiac event, and benefit improved overall health through benefits such as:

  • Convenient access to your physician and CCM care team through a designated phone number
  • Multiple forms of communicating such as texting, video conferencing, and emails to help improve the coordination of medical care
  • Improves quality of life by offering unique services such as social services and nutritional coaching
  • Helps to reduce hospital admissions/readmissions
  • Reduces the need for frequent travel to in-person doctor visits, or trips to the emergency room
  • Frequent medication reviews to adjust and validate they are being taken appropriately
  • Provides continuity of care through a designated nurse navigator

Remote Device Management And Monitoring

What is Remote Monitoring?

With remote cardiac monitoring, your cardiology team is alerted to cardiac abnormalities, allowing a quick response, and reducing the potential need for emergency care and hospitalizations. Cardiac remote monitoring can be utilized for patients who have pacemakers, defibrillators, implantable loop recorders, and CardioMEMS. This allows the saved data from your cardiac device to be viewed remotely by the clinicians in our office.

What is Device Management?

Implanted cardiac devices such as pacemakers, implantable cardioverter defibrillators, biventricular devices, and implanted cardiac loop recorders require ongoing management. Because the technology for devices that manage heart rhythm disorders is constantly evolving, our cardiology team provides ongoing care to ensure your device is maintained, working properly, and providing the support that you need.
